How do I replace a lost driver’s license in Ohio

You can apply for a license replacement in Ohio by going to the BMV and applying for it in person. Here’s how: 

Step 1: Provide these documents/information to start the replacement process- your driver’s license number, the type or class of driver’s license you own, your proof of identity and proof of residency, your date of birth, and your SSN.   

Step 2: Visit the nearest BMV office in Ohio to replace your driver’s license. Remember, you cannot replace a driver’s license in Ohio online. For that, you will have to go to the BMV in person.   

Step 3: A document of acknowledgement is necessary if your license is lost or stolen. You can also obtain a notarized affidavit in which you consent to surrender the license if it’s ever found.   

Step 4: The office will cross-check your documents with the information in their database and eventually approve a duplicate license.    

Step 5: Pay the replacement fee of $26 to get your duplicate license from the BMV.
