How do I replace a lost driver's license in MA?
You can replace a lost driver's license in Massachusetts either online, in person, or by phone. Here's how;
To replace it online:
- Log in to Massachusetts RMV website
- Enter your license permit or ID number, date of birth, and the last four digits of your SSN.
- Upload a photo and proof of address.
- Finally, pay a replacement fee of $25
To replace it in person:
- Visit your nearest local RMV.
- Fill in Form LIC100 (Driver’s license, learner’s permit, or ID card application form).
- Submit the documents supporting your residency, like any document issued by the RMV, a bank statement or credit card statement dated within 60 days, first-class mail, or federal, state, or municipal documents.
- As proof of your social security number, a social security card or a W-2 form can be submitted.
- Pay the fee of $25
To replace it by phone:
- Call the RMV’s Contact Center to request a replacement driver’s license.
- If you’re calling from area codes 339, 617, 781, and 857, or from outside Massachusetts, you can call this number – (857) 368-8000. All other area codes in MA can contact the RMV through this number – (800) 858-3926. For people who are deaf or hard-of-hearing (in Massachusetts), call TTY: (877) 768-8833.
- You will also need to provide the following information: First and last name, date of birth, and the last four digits of your Social Security number.
